This is a duplex refeed jam in the document feeder. The paper jam occurs on the back-side copy. The duplex refeed does not make it to the deskew sensor.
Remedy:
1. Remove the jammed paper. 2. Verify there is nothing in the paper path of the duplex refeed area. 3. Retry the copy job. 4. Replace the document feeder.
